Job summary

FOX Rehabilitation seeks a licensed Speech Language Pathologist to provide one‑on‑one care in the homes of older adults across New Jersey. You will evaluate, plan, and deliver evidence‑based therapy within our Geriatric House Calls model, with optional telehealth for flexibility.

You’ll enjoy autonomy over your schedule, collaborate with an interdisciplinary team, and document in FOX’s EMR to support patient independence and quality of life in private residences or senior living settings.

Qualifications

• Valid state licensure as Speech Language Pathologist.
• Current CPR certification and ASHA CCC.
• Proficiency with EMR documentation.

Responsibilities

• Provide one-on-one Speech Language Pathology to older adults in homes or senior living communities.
• Perform evaluations, develop individualized care plans, and deliver evidence-based therapy.
• Deliver sessions that support wellness, mobility, and functional independence.
• Document in FOX EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
