FOX Rehabilitation is expanding its geriatrics-focused Physical Therapy team in New Jersey, offering in-home and senior living visits. You’ll tailor one-on-one care plans for older adults, advancing mobility and quality of life through evidence-based therapy.

Role emphasizes autonomy in scheduling, collaboration with an interdisciplinary team, and optional telehealth to extend access and continuity of care.

Qualifications

• Licensed Physical Therapist in New Jersey or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Proficiency with EMR systems and telehealth tools.
• Current CPR certification and basic computer skills.

Responsibilities

• Provide one-on-one PT to older adults in-home or in senior living settings.
• Perform evaluations, develop care plans, and deliver evidence-based therapy.
• Deliver sessions that promote mobility, independence and safety at home.
• Document in FOX’s EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and fitness specialists.
• Educate patients, caregivers, and staff to promote safe aging in place.
• Offer telehealth/virtual care when appropriate.
