Job summary
FOX Rehabilitation is seeking licensed Physical Therapists to deliver one-on-one geriatric care in homes and senior living communities across New Jersey, with telehealth options available to extend access and continuity of care.
Therapists will evaluate, plan and implement evidence-based treatment, collaborating with an interdisciplinary team and educating patients and caregivers to promote aging in place and functional independence.
Qualifications
- Valid Physical Therapy license in the state of practice or eligibility to obtain
- Basic computer and EMR proficiency
- Current CPR certification
Responsibilities
- Provide one-on-one physical therapy to older adults in home or community settings
- Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment
- Deliver therapy sessions that support wellness, mobility and independence
- Document effectively within FOX’s EMR system per regulatory standards
- Collaborate with an interdisciplinary team of PTs, OTs, SLPs, and Fitness Specialists
- Educate patients, caregivers, and senior living staff to promote safety, function and aging in place
- Provide telehealth / virtual care when appropriate to extend access and flexibility
