Job summary

FOX Rehabilitation is expanding its Geriatric House Calls™ model in Downers Grove, IL, seeking licensed Physical Therapists who value autonomy and meaningful patient outcomes. You’ll manage your own schedule while treating older adults at home or in senior living communities, with optional telehealth components to extend access and continuity of care.

Qualifications

• Must hold a valid Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program.
• Proficiency with basic computer use and EMR systems.
• Current CPR certification is required.

Responsibilities

• Provide one-on-one physical therapy to older adults wherever they call home (private residences, senior living communities, etc).
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support patient wellness, mobility, and functional independence.
• Document effectively within FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
