Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in Highland Park, IL, including private residences and Senior Living Communities.

You will conduct evaluations, create individualized care plans, and provide evidence-based therapy sessions aimed at improving mobility, independence, and quality of life through our Geriatric House Calls model.

Qualifications

• Valid Physical Therapy license in the state of practice, or eligibility to obtain.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living settings.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively in FOX’s EMR system per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients, caregivers, and staff to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
