Job summary

Optimal Care is seeking a Registered Nurse to provide skilled nursing at patients' homes, delivering compassionate care in both Home Health and Hospice settings. This role emphasizes autonomy, patient connection, and meaningful impact in the communities we serve.

You will manage a caseload, conduct comprehensive assessments, develop individualized care plans, and coordinate with physicians, families, facilities, and an interdisciplinary team, with daily travel across Kalamazoo-area service sites.

Qualifications

• Graduate of an accredited Diploma, Associate or Baccalaureate School of Nursing.
• Current State License as a Registered Nurse.
• Minimum one (1) year experience as a nurse in a clinical care setting.
• Reliable transportation, valid driver's license, and valid automobile insurance coverage.

Responsibilities
