Job summary

Preferred Hospice of Missouri is seeking a dedicated RN Case Manager to serve patients in the Ozark, MO area. The role involves visiting patients in homes and facilities, providing comprehensive assessments, symptom management, and education to families.

You will supervise LPNs and CNAs, coordinate care, and ensure medications and DME needs are addressed, with a focus on compassionate, high-quality hospice care.

Qualifications

• Licensed RN in Missouri required.
• Must live within commuting distance to Ozark office.
• Proficient in EMR charting and using smartphone apps.
• Reliable transportation and valid driver’s license required.
• Full-time schedule Mon–Fri 8am–5pm; some travel to patient homes.

Responsibilities

• Conduct head-to-toe assessments in patients' homes, ALFs, SNFs, or other settings.
• Provide symptom management and education on death and dying to patients and families.
• Supervise LPNs and CNAs; monitor medications and assess DME needs; provide wound care as needed.
