• Ability to perform head-to-toe assessments and wound care.
• Proficient in EMR charting and using mobile apps for mileage.
• Willingness to travel to patients' homes within the service area.

Responsibilities

• Visit patients in private homes and facilities within assigned geography.
• Provide symptom management, education, and emotional support.
• Supervise LPNs and CNAs; monitor medications and DME needs.
• Coordinate care and maintain thorough documentation.
