Job summary

Preferred Hospice is seeking a Full Time RN Case Manager to serve patients in the Farmington, MO area. You will drive between visits, providing comprehensive in-home assessments and symptom management.

You will supervise LPNs and CNAs, coordinate care within a defined geographic area, and offer emotional support to patients and families facing terminal illness. Autonomy and teamwork are valued, with office meetings occasionally required.

Qualifications

• Licensed RN in Missouri
• Compassionate, patient-centered care
• Proficient in EMR charting and mobile apps
• Able to work in a team and maintain professionalism
• Reliable transportation and car insurance

Responsibilities

• See patients in private homes, assisted living facilities, skilled nursing facilities, or other patient homes.
• Perform head-to-toe assessments, manage symptoms, and provide education on terminal illness.
• Supervise LPNs and CNAs, monitor medications, assess DME needs, and perform wound care as needed.
