Job summary

Interim HealthCare Home Health and Hospice is seeking a Hospice RN Case Manager to provide skilled nursing care in patients’ homes, with autonomy to manage your day and deliver compassionate, one-on-one care. You’ll partner with physicians and CNAs to support families and coordinate care in a home setting.

The role requires an active MI RN license, at least 1 year of RN clinical experience (Hospice preferred), CPR certification, and reliable transportation.

Qualifications

• RN with active, unrestricted license in Michigan; BSN not required.
• At least one year of clinical RN experience (prefer Hospice).
• Current CPR certification.
• Reliable transportation with valid driver's license and car insurance.

Responsibilities

• Provide home-based care to patients with terminal illness.
• Work with a team of physicians and oversee CNAs providing patient care.
• Examine patients, perform assessments, document and report changes to their physician.
• Educate patients on their plan of care, prescribed medication, diet and exercise.
• Coach family members on the patient’s plan of care, medication and home safety.
• Coordinate discharge from services and ensure goals are met.
