Job summary

Interim HealthCare Home Health and Hospice in Holland, MI is seeking an experienced Hospice RN Case Manager to deliver compassionate, home-based nursing care with autonomy and balanced work-life.

As a nurse, you will work with a team to assess patients, educate families, coordinate care, and oversee CNAs while maintaining an unrestricted Michigan RN license and CPR certification. The position offers a salary with weekly bonus opportunities and comprehensive benefits.

Qualifications

• Registered Nurse with active, unrestricted license in Michigan (BSN not required).
• At least one year of clinical RN experience ( Hospice preferred).
• Current CPR certification.
• Reliable transportation with a valid state driver's license and car insurance.

Responsibilities

• Provide home-based care to patients with terminal illness.
• Work with a team of physicians and oversee CNAs providing patient care.
• Examine patients, perform assessments, document and report changes.
• Educate patients on plan of care, medications, diet and exercise.
• Coach family members on the plan of care, medications and home safety.
• Coordinate discharge from services.
