Job summary

The Care Team Hospice is seeking a part-time Nurse Practitioner to care for terminally ill patients in Crown Point, Indiana and surrounding areas. The NP will deliver hospice services primarily in patients' homes and facilities, coordinating with the IDG to tailor care plans and ensure quality, compassionate care.

Responsibilities include regular patient assessments, timely Face-to-Face visits, on-call coverage, and comprehensive documentation consistent with HIPAA.

Qualifications

• Licensed as a Nurse Practitioner in Indiana.
• RN license and NP license required.
• Graduate of an accredited nursing program.
• Minimum 2 years of RN experience preferred.

Responsibilities

• Provide hospice nursing services at patients' homes.
• Assess and reassess nursing needs; perform Face-to-Face visits timely.
• Develop and adjust the patient care plan with IDG.
• Maintain HIPAA-compliant patient records and documentation.
• Participate in on-call coverage as required.
