Residential Home Health and Hospice in Moosic, PA, is seeking a licensed practical nurse (LPN) to join our hospice team delivering end-of-life care in patients' homes. You will work 40 hours per week in a patient‑centered, autonomous role with interdisciplinary collaboration.

Ideal candidates hold an active LPN license, have 2+ years of LPN experience (hospice preferred), and can travel within the territory.

Qualifications

• Active LPN license in the state you work.
• Two years of LPN experience; hospice experience preferred.
• Driver’s license and ability to spend ~20% of time driving.
• Drug screen prior to employment.

Responsibilities

• Deliver compassionate, patient-centered care in a hospice setting.
• Conduct ongoing patient assessments focused on comfort and symptom management.
• Educate patients and caregivers on end‑of‑life care.
• Ensure thorough EMR documentation.
• Work independently, but collaborate with an interdisciplinary team.
• Travel to patient homes within assigned territory.
