Residential Home Health and Hospice is a nationally recognized leader in home health and hospice, offering autonomy, meaningful patient relationships, and real career growth. We are hiring a Licensed Practical Nurse for a Hospice position delivering end-of-life care in patients' homes in Moosic, PA.

You will provide compassionate, patient-centered hospice care, collaborate with an interdisciplinary team, educate patients and caregivers on symptom relief, and ensure accurate EMR documentation.

Qualifications

• Active LPN license in the state you work.
• Two years of LPN experience; Hospice experience preferred.
• Valid driver’s license and ability to spend ~20% of workday driving to patient locations.

Responsibilities

• Deliver compassionate, patient-centered hospice care with the interdisciplinary team.
• Conduct ongoing assessments focused on comfort and symptom management.
• Educate patients and caregivers on end-of-life care and relief strategies.
• Ensure thorough, compliant EMR documentation.
• Work independently, but stay connected with the team.
• Travel to patient homes within assigned territory.
