Graham Healthcare Group is hiring a Licensed Practical Nurse for a Hospice position delivering end-of-life care to patients in their homes around Moosic, PA. You will work with an interdisciplinary team to manage comfort, symptoms, and changes in condition, while educating patients and caregivers on what to expect during the hospice journey.

This field-based role requires a valid LPN license, driving about 20% of time, and a strong commitment to compassionate care; we offer education pathways

Qualifications

• Active LPN license in the state you work
• Two years of LPN experience and the clinical expertise to deliver quality patient care
• Due to field-based nature, valid driver’s license and ability to travel ~20% of workday
