Job summary

Brooks Home Health in Jacksonville, FL is seeking an experienced Registered Nurse to plan and manage patient care in collaboration with a multidisciplinary team. The role emphasizes timely care planning, documentation, and patient education to promote wellness and independence.

The nurse will develop individualized plans within 24 hours of admission, coordinate with the rehabilitation team, and participate in on-call and weekend rotations as needed.

Qualifications

• Registered Nurse license required in Florida.
• BLS certification required and must be maintained.
• Experience in admission planning, discharge planning, and patient education preferred.

Responsibilities

• Identify nursing/clinical problems via bio-psycho-social assessment.
• Develop and document comprehensive individualized care plans within 24 hours of admission.
• Plan and coordinate care with the multidisciplinary team across shifts.
• Administer medications and treatments as ordered, monitor responses and adverse effects.
• Complete OASIS per guidelines and ensure accurate documentation.
• Provide patient and family education and discharge planning.
• Collaborate with the rehabilitation team to promote wellness and independence.
