Job summary

Brooks Home Health in Jacksonville, FL seeks a Registered Nurse to plan and manage patient care in collaboration with a multidisciplinary team. In this non-supervisory role, you will develop comprehensive care plans and coordinate with clinicians to promote independence and wellness for patients with disabilities or chronic conditions.

You will document outcomes, participate in care conferences, and ensure adherence to nursing standards.

Qualifications

• Registered Nurse license issued by the State of Florida.
• Basic Life Support (BLS) certification.
• Bachelor’s, Master’s or equivalent nursing degree welcome.

Responsibilities

• Utilizes nursing process to assess and identify problems.
• Develops individualized care plans within 24 hours of admission.
• Coordinates with multidisciplinary team to achieve goals.
• Documents care and updates plans based on progress.
• Performs medication administration and reconciliation.
• Maintains patient safety and quality of care.
• Engages in patient and family teaching and discharge planning.
• Participates in on-call/weekend rotations as needed.
