Job summary

Brooks Home Health in Gainesville, FL is seeking a Registered Nurse to manage patient care within a multidisciplinary team. This role emphasizes assessment, care planning, and patient education to promote independence and wellness.

You will collaborate with families, rehab staff, and physicians to develop individualized plans, administer medications, document accurately, and participate in discharge planning and ongoing education for patients and caregivers.

Qualifications

• Registered Nurse with a State of Florida license.
• Associate, Bachelor, or Master’s degree in Nursing from an accredited school.
• BLS certification required; maintain ongoing education and credentials.

Responsibilities

• Identify actual/potential nursing problems from assessments and document findings.
• Develop and implement a comprehensive nursing plan of care within 24 hours.
• Collaborate with clients, families, and rehab team to set wellness-oriented goals.
• Administer medications and treatments, monitor responses, and report adverse effects.
• Complete medication reconciliations and coordinate with the team.
• Document per guidelines and participate in discharge planning and teaching.
• Complete OASIS according to guidelines and ensure timely updates.
