Qualifications

• Admission Nurse background preferred.

Responsibilities

• Develops and documents individualized nursing plans of care.
• Completes medication reconciliation at SN visits and communicates changes to the team.
• Administers medications and treatments; monitors patient response.
• Performs and documents OASIS per guidelines.
• Collaborates with interdisciplinary team to optimize outcomes.
• Precepts and supports orientation for new staff.
