Job summary

VitalCaring is seeking a Registered Nurse (RN) for Home Health in the Joplin, Missouri area. As a Home Health RN, you will serve as the clinical lead, managing plans of care, coordinating with interdisciplinary teams, and driving outcomes from admission through discharge.

You will perform advanced assessments, care planning, and patient education to reduce hospitalizations and promote recovery at home. A field-based role offers autonomy with a supportive clinical team to back you up.

Qualifications

• Active RN license (or compact) + CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health or related).
• Strong clinical judgment and ability to think independently.
• Excellent communication and patient-centered approach.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct in-home assessments to evaluate clinical status and goals of care.
• Develop and manage individualized care plans in collaboration with physicians.
• Deliver skilled nursing care including medications and treatments.
• Serve as case manager coordinating across therapists, aides and physicians.
• Monitor patient progress and adjust care plans as conditions change.
• Educate patients and families to support disease management and independence.
• Coordinate transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
