Job summary

VitalCaring Group in Missouri is seeking a Field-Based Home Health RN to lead plans of care, coordinate across disciplines, and drive outcomes from admission through discharge. You’ll deliver skilled nursing, educate families, and manage a caseload with autonomy in patients’ homes.

This role requires an active RN license, CPR certification, and at least 1 year of RN experience; proficiency with EMR systems and a valid driver’s license are also essential.

Qualifications

• Active RN license (or compact eligibility) + CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU or Acute Care preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication skills and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status and goals.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care, including medications and treatments.
• Serve as the case manager coordinating across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions as conditions change.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
