VitalCaring is seeking a Field-Based Home Health RN to lead patient care in a defined territory in Missouri. You will perform in-home assessments, develop care plans, provide skilled nursing services, and coordinate with physicians and the care team to drive outcomes.

This role offers autonomy within a supportive clinical network and opportunities to impact recovery at home. Ideal candidates have an active RN license, CPR, at least 1 year in home health or acute care, strong clinical judgment,

Competenze

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and ability to think independently, with excellent communication.
• Proficiency with EMR systems and valid driver's license with reliable transportation.

Mansioni

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care
• Develop and manage individualized care plans in collaboration with physicians and care teams
• Deliver skilled nursing care, including medication management, treatments, and interventions
• Serve as the case manager, coordinating care across therapists, aides, and physicians
• Monitor patient progress and make clinical decisions based on changing conditions
• Educate patients and families to support disease management and independence
• Coordinate care transitions and discharge planning to reduce readmissions
• Maintain timely, accurate documentation in the EMR
• Supervise and support Home Health Aides as needed
• Participate in on-call rotation as appropriate
