VitalCaring Group in Gautier, Mississippi, is seeking a Registered Nurse (RN) to lead home health care with clinical autonomy and strong patient outcomes. The PRN/home-based role emphasizes advanced assessment, care planning, and collaboration with a multidisciplinary team.

You’ll manage plans of care from admission through discharge, build meaningful patient relationships, and thrive in a flexible, autonomous field setting with a supportive leadership team.

Formación

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health preferred).
• Strong clinical judgment and ability to think independently.
• Excellent communication and patient-centered approach.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsabilidades

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans in collaboration with physicians and care teams.
• Deliver skilled nursing care, including medication management, treatments, and interventions.
• Serve as case manager, coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise and support Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
