Job summary

VitalCaring Group is seeking a Field-Based RN to lead in-home patient care, delivering skilled nursing, planning individualized care, and coordinating with physicians and therapists. You will assess, treat, and educate patients in their homes to improve recovery and prevent hospitalisations.

Ideal candidates have an active RN license, 1+ year home health experience, strong clinical judgment, and excellent communication skills. A valid driver’s license and EMR proficiency are essential.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum 1 year RN experience (Home Health, Med-Surg, ICU, or Acute Care).
• Strong clinical judgment and independent thinking.
• Excellent communication and patient-centered approach.
• Proficient with EMR systems.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care including medication management and treatments.
• Act as case manager coordinating across therapists, aides, and physicians.
• Monitor patient progress and adjust care based on changing conditions.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely EMR documentation.
• Supervise Home Health Aides and participate in on-call rotation.
