Job summary

VitalCaring is seeking a Home Health Registered Nurse to serve patients in their homes within an assigned territory. As the clinical lead, you will conduct in-home assessments, develop care plans, and deliver skilled nursing care.

You will work with a collaborative clinical team, exercise strong clinical judgment, manage a caseload, document in the EMR, and participate in on-call rotations. A valid RN license and CPR are required.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum one year RN experience (Home Health, Med-Surg, ICU, or Acute Care preferred).
• Strong clinical judgment and independent thinking.
• Excellent communication skills and patient-centered approach.
• Proficiency with EMR systems.
• Valid driver’s license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care, including medication management and interventions.
• Coordinate care across therapists, aides, and physicians as case manager.
• Monitor patient progress and adjust care plans based on conditions.
• Educate patients and families on disease management and independence.
• Document in the EMR in a timely and accurate manner.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
