Job summary

VitalCaring is seeking a Home Health RN to be the clinical lead for your patients—managing the plan of care, coordinating across disciplines, and driving outcomes from admission through discharge. You’ll apply advanced assessment, care planning, and patient education to support recovery and independence at home.

This field-based role offers autonomy within a collaborative clinical team, with a focus on high-impact care.

Qualifications

• Active RN license (or compact eligibility) and CPR certification.
• Graduate of an accredited nursing program.
• Minimum of 1 year RN experience; home health, Med-Surg, ICU, or acute care preferred.
• Strong clinical judgment and independent thinking.
• Excellent communication and patient-centered approach.
• Ability to manage a caseload and prioritize effectively.
• Proficiency with EMR systems.
• Valid driver's license and reliable transportation.

Responsibilities

• Conduct comprehensive in-home assessments to evaluate clinical status, risks, and goals of care.
• Develop and manage individualized care plans with physicians and care teams.
• Deliver skilled nursing care, including medication management and interventions.
• Serve as case manager coordinating care across therapists, aides, and physicians.
• Monitor patient progress and make clinical decisions based on changes in condition.
• Educate patients and families to support disease management and independence.
• Coordinate care transitions and discharge planning to reduce readmissions.
• Maintain timely, accurate documentation in the EMR.
• Supervise Home Health Aides as needed.
• Participate in on-call rotation as appropriate.
