Job summary

Optum's Caretenders division is hiring a Home Health Registered Nurse to deliver skilled nursing in patients' homes across Northeast Central Indiana, including Wabash, Grant, Huntington, and Blackford Counties. You will assess patients, coordinate with physicians and the care team, and ensure safe, high-quality care while fostering strong relationships with patients and families.

The role offers full-time hours, per-visit pay, and benefits for eligible employees.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR Certification.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provide and direct nursing care in home as ordered by physician and per regulations.
• Complete initial and comprehensive nursing assessments.
• Develop, implement and revise patient plans of care with physicians and team.
• Perform OASIS assessments and medication reconciliation.
• Coordinate care with patients, families, physicians and interdisciplinary team members.
• Provide high quality clinical care within scope of practice and infection control standards.
• Supervise home health aides and LPN/LVNs per regulations.
• Ensure timely and accurate documentation.
• Support patient safety initiatives and positive clinical outcomes.
• Participate in orientation and mentoring activities; participate in on-call and weekend rotation.
