Job summary

Caretenders, a care brand within the Optum family, is seeking a Registered Nurse for home health in the Boston area. The role involves directing nursing care in patients’ homes, coordinating the plan of care with physicians and the care team, and ensuring compliance with laws and agency policies.

Required is an active RN license and a valid driver’s license with reliable transportation. Responsibilities cover supervision, assessments, documentation, and collaboration with families and clinicians

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.

Responsibilities

• Initiates, develops, implements, and revises the plan of care with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical/vocational nurses, provides instruction, and assigns tasks according to state and federal regulations.
• Provides required supervisory visits.
• Completes comprehensive assessments (OASIS) including medication reconciliation accurately and timely.
• Documents patient visits per policy and payer requirements, and syncs timely per LHC policy.
• Communicates relevant information timely with appropriate staff and families to ensure quality care and service.
• Participates in on-call and weekend rotations as needed.
