Job summary

UnitedHealth Group is seeking a Home Health Registered Nurse for the Greater Indianapolis area to deliver skilled nursing care in patients’ homes. You will assess, plan, implement and coordinate care with physicians and the interdisciplinary team, while promoting safety and quality outcomes.

This role offers flexible employment types (FT/PT), a per-visit compensation structure, and a $10,000 sign-on bonus for eligible full-time external hires.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• 1+ years of Home Health experience.
• Current CPR Certification.
• Valid Driver's License, vehicle insurance, and access to dependable transportation.

Responsibilities

• Complete initial and comprehensive nursing assessments.
• Develop, implement and revise patient plans of care with physicians and other healthcare professionals.
• Perform OASIS assessments and medication reconciliation.
• Coordinate care with patients, families, physicians and interdisciplinary team members.
• Provide high quality clinical care within the scope of practice and infection control standards.
• Supervise home health aides and licensed practical or vocational nurses in accordance with regulations.
• Ensure timely and accurate documentation.
• Support patient safety initiatives and positive clinical outcomes.
• Participate in orientation and mentoring activities as needed.
• Participate in on-call and weekend rotation as needed.
