Job summary

Caretenders, a part of LHC Group and a member of the Optum family, operates within UnitedHealth Group and cares for patients in their homes. We seek a Registered Nurse in Home Health to deliver and direct nursing care as prescribed and coordinate with physicians and the care team.

As part of a dedicated team, you will ensure compliance with laws and agency policies while supporting continuity of patient care and improving health outcomes in the community.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR Certification or ability to complete within 90 days of hire

Responsibilities

• Initiates, develops, implements, and revises the plan of care in collaboration with the physician and other health care professionals.
• Supervises care provided by home health aides and licensed practical nurses; provides instruction and assigns tasks according to regulations.
• Provides supervisory visits and coordinates with the care team to ensure plan of care is followed.
• Completes comprehensive assessments (OASIS) and medication reconciliation, timely and accurately.
• Documents patient visits per policy and payer requirements, syncing per agency policy.
