Job summary

Optum's Indiana Homecare Northwest is seeking a Home Health Registered Nurse to deliver skilled nursing in patients' homes across Lake and Laporte Counties. You will build relationships with patients and families, coordinate with physicians, and guide care plans with the interdisciplinary team to help patients achieve health goals.

This role offers autonomy, meaningful impact, and a per-visit pay structure with a $10,000 sign-on bonus for eligible full-time external hires.

Qualifications

• Current and unrestricted RN licensure in state of practice.
• Current Driver's License, vehicle insurance, and access to a dependable vehicle or public transportation.
• Current CPR Certification.
• Ability to function in any home situation regardless of age, race, creed, color, sex, disability, or financial condition of the client.

Responsibilities

• Provide and direct nursing care in the home as ordered by the physician and in accordance with regulations and agency policies.
• Complete initial and comprehensive nursing assessments.
• Develop, implement and revise plans of care with physicians and other professionals.
• Perform OASIS assessments and medication reconciliation.
• Coordinate care with patients, families, physicians and team members.
• Provide high quality clinical care within scope of practice and infection control standards.
• Supervise home health aides and licensed staff according to regulations.
• Ensure timely and accurate documentation.
• Support patient safety initiatives and positive clinical outcomes.
• Participate in orientation and mentoring activities; on-call and weekend rotation as needed.
