Job summary

Elara Caring is seeking a Home Health Registered Nurse (RN) to provide one-on-one, in-home care in Worcester, MA. You will manage a dedicated caseload with physician-guided plans of care and a supportive clinical team.

Qualifications

• Current, unrestricted RN license.
• Associate degree in nursing required; BSN preferred.
• At least one year of clinical nursing experience.
• Home health experience is required.
• Strong assessment, communication and care-coordination skills.

Responsibilities

• Assess patients and develop individualized plans of care.
• Provide skilled nursing services per physician orders.
• Monitor patient conditions, medications and progress.
• Educate patients and caregivers on treatments and disease management.
• Coordinate care with physicians and the interdisciplinary team.
• Maintain accurate and timely clinical documentation.
