Job summary

Elara Caring is seeking a Home Health Registered Nurse in Chicopee/Springfield territory. You will provide one-on-one care at patients’ homes, manage caseloads, and coordinate with clinicians to support independence and safety.

Qualifications

Current, unrestricted RN license.
Associate degree in Nursing required; bachelor’s degree preferred.
At least one year of clinical nursing experience.
Willingness to travel throughout the assigned territory.
Previous home health or hospice experience preferred.
Homecare Homebase experience preferred.
Dependable vehicle, valid driver’s license, and current auto insurance.
Ability to lift 50–100 lbs.

Responsibilities

Manage an assigned caseload of home health patients.
Complete comprehensive assessments and ongoing evaluations.
Develop, implement, and update individualized plans of care.
Coordinate services with physicians, therapists, caregivers, and clinical leadership.
Educate patients and families on medications, treatments, disease management, and safety.
Monitor changes in each patient’s condition and communicate significant findings.
Maintain accurate, timely clinical documentation.
Help patients achieve better outcomes while remaining safely at home.
