Job summary

Elara Caring in Worthington, Ohio is seeking a Registered Nurse Home Health to join our dedicated team. This staff RN role focuses on delivering high-quality in-home care to patients in their residences, with autonomy and flexibility in scheduling.

As part of our mission, you will assess patient needs, collaborate with the care team, and adjust plans of care while maintaining HIPAA privacy and safety standards. Travel up to 50% is required.

Qualifications

• Active RN license in Ohio (or state of residence) and in good standing.
• At least 1 year of clinical nursing experience.
• Willing to travel up to 50% within the branch coverage area.
• Valid driver’s license, reliable vehicle, and auto insurance.
• Preferred: Associate or Bachelor’s degree in Nursing; Home Health or Hospice experience.

Responsibilities

• Care for assigned home health patients and families in their residence.
• Develop, implement, and evaluate plan of care with Interdisciplinary Team.
• Communicate changes in patient condition to the supervisor and care team.
• Ensure privacy and HIPAA compliance during patient care.
• Travel to patient homes and coordinate care logistics.
