Job summary

VNA Health in Santa Barbara, CA is seeking a compassionate Physical Therapist for Home Health. You will deliver personalized rehabilitative care in patients' homes, supporting mobility and independence through evidence-based therapy and patient education.

Qualifications

• California Physical Therapist license required.
• Minimum of two years of physical therapy experience; home health experience preferred.
• Current CPR certification.
• Valid driver’s license with insured vehicle.
• Excellent verbal and written communication and organizational skills.

Responsibilities

• Provides physical therapy services to patients according to a written physician's plan of care.
• Assesses muscle strength, mobility, gait, ROM and rehab potential; evaluates home environment.
• Directs physical therapy treatment and instructs patients and families in use and care of therapeutic appliances.
• Determines priority needs for physical therapy and reports patient progress to physician.
• Instructs other personnel and family members in therapy goals and may train patients in prosthetic device use.
