Job summary

VNA Health is seeking a dedicated Physical Therapist to join our Home Health team serving the Santa Ynez Valley. You will deliver high-quality rehabilitative care in patients' homes, emphasizing autonomy and clinical decision making within an interdisciplinary team.

The role offers a full-time schedule, with 40 hours per week and rotating weekend shifts. California PT licensure and CPR certification are required, along with strong communication and organizational skills.

Qualifications

• CA licensed Physical Therapist required.
• Two years of PT experience; home health preferred.
• Current CPR certification required.
• Licensed driver with insured automobile.
• Strong verbal and written communication and organizational skills.

Responsibilities

• Provide PT services per physician's plan of care and home setting.
• Evaluate functional status, mobility, and ROM; adjust care as needed.
• Instruct patients and family in use of therapeutic devices and exercises.
• Supervise home health aides and PT assistants as needed.
• Prepare clinical/progress summaries and meet productivity standards.
