VNA Health seeks a compassionate Physical Therapist to join our Home Health team serving the Santa Ynez Valley. Provide personalized home-based rehabilitative care with emphasis on mobility and independence. Regular full-time schedule with rotating weekends, holidays as needed.

The role emphasizes autonomous clinical decision-making within a collaborative interdisciplinary team, offering opportunities for growth and meaningful patient impact.

Formación

• Current and active California Physical Therapist license.
• Two years of appropriate experience as a physical therapist.
• Current CPR certification.
• Licensed driver with insured automobile.
• Demonstrates good verbal and written communication and organization skills.

Responsabilidades

• Provide physical therapy services to patients per physician's plan of care.
• Assess functional status, mobility, gait, ROM; participate in care planning.
• Instruct patients and families in use and care of therapeutic devices.
• Direct physical therapy treatment and supervise aides when applicable.
• Prepare clinical progress summaries and report patient progress.

California Physical Therapist license
