Job summary

VNA Health is seeking a compassionate Physical Therapist to join our Home Health team serving Santa Barbara. This role offers autonomy, clinical decision-making, and the opportunity to deliver high-quality rehabilitative care in patients' homes.

You will hold a California PT license, CPR certification, and bring 2 years of PT experience, preferably in home health. The position is full-time, 40 hours weekly, with rotating weekend shifts and holidays as needed.

Qualifications

• Current and active California Physical Therapist license.
• Two years of PT experience; home health experience preferred.
• Current CPR certification.
• Licensed driver with insured automobile.
• Demonstrates good verbal and written communication and organization skills.

Responsibilities

• Provides physical therapy services per physician's plan of care.
• Assesses functional status and home environment to plan care.
• Instructs patients and caregivers in use of therapeutic devices.
• Supervises home health aides and PT aides as needed.
• Documents clinical progress and communicates with the care team.
