Job summary

Residential Home Health and Residential Hospice is seeking a Home Health Physical Therapist to provide skilled care to patients in Belleville and surrounding areas. This full-time role offers autonomy, meaningful patient relationships, and opportunities for growth, with a $10,000 first-year success bonus.

You’ll perform evaluations, develop therapy plans, educate patients and caregivers, and document in EMR.

Qualifications

• Active Physical Therapist license in the state you work.
• One year of experience as a Physical Therapist.
• Valid driver's license and ability to travel ~20% of time.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you
• Perform comprehensive patient evaluations, functional mobility assessments, and develop individualized therapy plans to improve strength, balance, and independence
• Educate and empower patients and caregivers on therapeutic exercises, injury prevention, and safe mobility strategies to support recovery and confidence at home
• Think critically, solve problems in real time, and ensure thorough, compliant EMR documentation
• Work independently, but never alone
• Travel to patient homes within assigned territory
