Qualifications

• Bachelor's or Doctoral degree in Physical Therapy from an accredited program.
• Current and unrestricted Physical Therapist license in the practicing state.
• Proven ability with clinical assessments, documentation, and policy compliance.
• Strong organizational and communication skills.
• Experience in Home Health Therapy; familiarity with OASIS preferred.

Responsabilités

• Conduct comprehensive home health assessments and develop individualized treatment plans.
• Deliver high-quality PT services in patients' homes.
• Collaborate with a multidisciplinary team to coordinate care.
• Monitor patient progress and adjust treatment plans as needed.
