Quality Healthcare Group, Inc. is seeking a licensed Physical Therapist to provide skilled, evidence-based therapy services to adults in home and residential settings.

The PT will evaluate functional mobility, design individualized treatment plans, and collaborate with interdisciplinary staff to maximize independence, safety, and quality of life. This role offers flexible scheduling, autonomy in clinical decision-making, and opportunities for program development and leadership within a home

Qualifications

• Doctor of Physical Therapy (DPT) or Master’s in Physical Therapy from an accredited program.
• Active Pennsylvania Physical Therapist license in good standing.
• CPR certification.
• Valid driver’s license and reliable transportation for community visits.
• Ability to work independently in home/community environments.

Responsibilities

• Perform initial PT evaluation including mobility, fall risk, strength, balance, pain, safety, and home environment.
• Establish measurable functional goals and a time-limited plan of care.
• Provide skilled interventions: gait training, therapeutic exercise, balance, pain management, device training.
• Instruct patients and caregivers in home exercise programs and safety strategies.
• Conduct periodic reassessments and document progress toward goals.
• Maintain timely, compliant documentation per Medicare CMS rules and agency policy.
• Participate in case conferences and interdisciplinary coordination.
