Job summary

Nursing & Rehab At Home is seeking a dedicated registered nurse to deliver hands-on nursing care in patients' homes across our service area. You will craft personalized care plans, administer medications, and coach families to manage health confidently.

Join a clinician-owned, family-run agency known for long-tenured staff and a supportive, nurturing environment. A valid CA RN license, CPR, and a reliable vehicle are required.

Qualifications

• A degree from an accredited school of nursing.
• A current state RN license and CPR certification.
• A valid driver's license and a reliable, insured car.
• Strong observation, communication (verbal and written), problem-solving, and basic math skills.
• The ability to do the hands-on parts of the job: standing and walking for long stretches, lifting supplies and equipment, helping patients move or transfer safely, and reaching, bending, and kneeling as needed.
• Good vision and hearing to perform nursing skills safely.

Responsibilities

• Get to know new patients and their families, and figure out what they need to thrive at home.
• Build personalized care plans with patients and families, not just for them.
• Provide hands-on nursing care, including medications and treatments as prescribed.
• Teach patients and families how to feel confident managing their health.
• Partner with physicians, community resources, and home health aides to keep care coordinated.
• Guide and support our home health aides.
• Help plan for a smooth transition when a patient is ready for discharge.
