Residential Home Health and Hospice is hiring an RN to provide skilled nursing in patients' homes around Villa Park, CA. You’ll partner with a collaborative team, perform assessments and care planning, and empower families with disease management education.

Responsibilities

• Deliver skilled, patient-centered care in a home health setting with an interdisciplinary team.
• Perform patient assessments, medication management, and care planning.
• Educate families on disease management and treatment plans for confidence and understanding.
• Think critically, solve problems, and ensure compliant EMR documentation.
• Work independently, but stay connected with the care team.
• Travel to patient homes within the assigned territory.
