Residential Home Health and Hospice in Evanston, IL, is seeking a Licensed Practical Nurse for a Home Health position delivering skilled nursing care to patients in their homes in the Evanston area.

This role offers a $10,000 first-year bonus to reward achievement of key milestones in the first year, with a 40-hour work week and supportive leadership that fosters growth and independence.

Qualifications

• Active LPN license in the practicing state.
• Minimum 2 years of LPN experience in clinical care.
• Current driver’s license and ability to travel ~20% of your day.

Responsabilités

• Deliver skilled, patient-centered care in a home health setting, in collaboration with an interdisciplinary team that truly supports you.
• Perform comprehensive patient assessments including vital signs, medication management, wound care, and overall health status.
• Educate and support patients and caregivers on disease management, medication administration and prescribed treatments to encourage adherence and confidence in care routines.
• Maintain accurate, timely, and compliant EMR documentation.
• Work independently, but never alone.
• Travel to patient homes within assigned territory.
