Job summary

Nason Hospital Home Health, part of LHC Group and within the Optum family, is seeking a dedicated Licensed Practical Nurse for full-time home health care. The role includes direct patient care in clients' homes, documentation, and care coordination with the team. A $10,000 sign-on bonus is available to new hires.

Responsibilities include implementing the plan of care, maintaining infection control, and communicating with patients and families. Must have valid LPN license and a driver's license.

Qualifications

• Current and unrestricted LPN licensure in state of practice.
• Current driver's license and vehicle insurance, access to a dependable vehicle, or public transportation.

Responsibilities

• Provides high quality clinical services within the scope of practice, as defined by state laws and the plan of care.
• Documents visits thoroughly in the EMR per regulatory and payer requirements.
