AHN Healthcare@Home is hiring a Licensed Practical Nurse for a Home Health position. You will provide skilled nursing care to patients in their homes across Clinton, Oakdale, and Coraopolis areas, with a $10,000 first-year success bonus for achieving key milestones.

Qualifications

• Active LPN license in the state you work.
• Two years of LPN experience.
• Current driver’s license and ability to spend ~20% of your day driving to/from patient locations.

Responsibilities

• Deliver skilled, patient-centered care in home health settings in collaboration with an interdisciplinary team.
• Perform comprehensive patient assessments including vital signs, wound care, and health status.
• Educate and support patients and caregivers on disease management and prescribed treatments.
• Maintain accurate EMR documentation.
