Job summary

Rehab Without Walls Neuro Rehabilitation is seeking an experienced Speech Language Pathologist to deliver functional rehabilitation in patients' homes and communities.

You will conduct evaluations, design treatment plans, and monitor progress to help clients regain communication, cognitive skills, and independence. This role emphasizes autonomous practice within a collaborative interdisciplinary team and prioritizes real-life functional outcomes.

Qualifications

• Master’s Degree in Speech Language Pathology from an accredited program.
• Current valid SLP license in the state(s) of practice.
• CPR certification current.
• At least one year of experience as an SLP.
• Experience with rehabilitation techniques for neurological injury preferred.
• Strong communication skills, professional verbal and written.
• Ability to lift 50 pounds and assist with transfers up to 100 pounds.
• Physical capability per safety policies.

Responsibilities

• Perform evaluations and develop treatment plans for patients with speech, language, cognitive and swallowing disorders.
• Regularly reassess effectiveness of treatment plans and participate in staffing meetings and family conferences.
• Perform discharge evaluations and provide home/community safety recommendations and resources.
• Communicate patient needs and progress to the treatment team, physician, individual served and family members.
• Collaborate with other disciplines to coordinate care and enhance patient outcomes.
