Job summary

Mass General Brigham Home Care, Inc. seeks a Registered Nurse to provide skilled nursing in patients' homes within Hyde Park and surrounding areas. You will assess, plan, educate, and coordinate care with an emphasis on patient-centered outcomes.

Hybrid schedule, with 5-6 visits per day and no evenings, offering strong team support, admin assistance, and ongoing professional development as part of the Mass General Brigham system.

Qualifications

• Graduate of an approved School of Nursing.
• Current Massachusetts Registered Nurse license.
• At least one year of acute care nursing experience (home care experience preferred).
• Comfort working with various computer systems; experience with handheld clinical devices preferred.
• Strong interpersonal and customer service skills with the ability to work with diverse populations.

Responsibilities

• Complete patient assessments and admissions.
• Develop and manage individualized care plans.
• Provide skilled nursing care, education, and treatment.
• Coordinate care with patients, families, and providers.
• Document care and review charts.
• Collaborate with the interdisciplinary team.
• Deliver high-quality, patient-centered care.
