Job summary

FOX Rehabilitation is seeking licensed Physical Therapists to provide one-on-one care to older adults in homes and senior living communities in Tappan, NY and surrounding areas. Clinicians may treat in private residences or within Senior Living Communities, with the option to incorporate telehealth for added flexibility.

The role emphasizes autonomy, flexible scheduling, and building lasting patient relationships while delivering evidence-based therapy and evidence-driven outcomes.

Qualifications

• Valid Physical Therapy license in the state(s) of practice.
• Degree from an accredited Physical Therapy program.
• Basic computer and EMR proficiency.
• Current CPR certification.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions to support wellness, mobility, and functional independence.
• Document in the EMR system per regulatory standards.
• Collaborate with a multidisciplinary team (PT, OT, SLP, Fitness Specialists).
• Educate patients and caregivers to promote safety and aging in place.
• Provide telehealth/virtual care when appropriate.
