Job summary

FOX Rehabilitation is seeking a licensed Physical Therapist to deliver one-on-one geriatric care in Algonquin, IL, and surrounding communities. You’ll build a flexible schedule around in-home, senior living, and virtual care visits to improve mobility, independence, and quality of life for older adults.

You’ll evaluate patients, develop care plans, document in our EMR, and collaborate with an interdisciplinary team, all while maintaining autonomy and strong patient relationships in a supportive,

Qualifications

• Valid Physical Therapy license in the state of practice.
• Degree from an accredited Physical Therapy program.
• Current CPR certification.
• Basic computer and EMR proficiency.

Responsibilities

• Provide one-on-one physical therapy to older adults in homes or senior living communities.
• Perform evaluations, develop individualized plans of care, and deliver evidence-based treatment.
• Deliver therapy sessions that support wellness, mobility, and independence.
• Document effectively in the EMR per regulatory standards.
• Collaborate with PTs, OTs, SLPs, and Fitness Specialists.
• Educate patients and caregivers to promote safety and aging in place.
